A Limited Liability Company was established as UHS of Doylestown, L.L.C.; entity #3699827; effective 1-5-2007 (legal name). The following d/b/a(s) were established: Foundations Behavioral Health System; entity #3745872; effective 7-27-2007 and Foundations Behavioral Health, entity #6976033; effective 11-14-2019. Cancellation of d/b/a Foundations Behavioral Health System, entity #3745872; was effective 8-20-2020. A new d/b/a application was filed for Foundations Behavioral Health System, with the assigned entity #7122227, effective 9-2-2020.
